Romatoid artrit’li hastalarda ICF temelli aktivite ve katılımın incelenmesi

Abstract

Bu çalışmanın iki amacı vardır. Birinci amacı RA’li katılımcılarda hastalık aktivitesi ile özür düzeyi arasındaki ilişkiyi incelemektir. Çalışmanın ikinci amacı ise özür değerlendirme ölçeklerinin her bir maddesini ICF bileşenleri ve ICF El Kısa Set’teki kategoriler ile ilişkilendirmektir. Bu çalışmaya RA tanısı almış, yaş ortalaması 49.93±11.38 yıl olan 100 katılımcı (86 kadın, 14 erkek) dahil edildi. Katılımcıların hastalık aktviteleri DAS28 ile; özür düzeyleri DASH, MHQS, DEİ ve AIMS2 ölçekleri ile değerlendirildi. Üç uzman tarafından özür ölçeklerinin maddeleri ICF El Kısa Set’in kategorileri ile ilişkilendirildi. DAS28 puanı ile DASH (r:0.643) ve DEİ (r:0.575) toplam puanları arasında orta düzey ilişki, MHQS toplam puan ve alt ölçekleri arasında zayıf-orta düzey (r:-0.399 ve r:-0.696) arasında değişen ilişki, AIMS2 alt ölçekleri arasında zayıf-orta düzey (r:0.220 ve r:0.638) arasında değişen ilişki bulundu. Özür değerlendirme ölçeklerinin her bir maddesini ICF bileşenleri ve ICF El Kısa Set’teki kategoriler ile ilişkilendiren üç araştırmacı arasındaki uyum düzeyi incelendiğinde Kappa değerinin 0.78-0.90 arasında değiştiği ve yüksek düzeyde olduğu görüldü. Özür ölçeklerindeki maddelerin ICF El Kısa Set’e göre aktivite ve katılımı kapsama yüzdeleri incelendiğinde %52.63 ile AIMS2’nin en yüksek değere sahip olduğu belirlendi. Bu çalışmanın sonuçları, RA’li hastalarda bozukluk düzeyinin özür düzeyini etkilediğini ve ICF El Kısa Set’deki aktivite ve katılım bileşenlerini özür değerlendirme ölçekleri arasında en çok AİMS2’nin kapsadığını göstermektedir.

There are two aims of this study. The first aim is to analyze the relationship between disease activity and disability level in patients with RA. The second aim of the study is to associate each item of the disability assessment scales with the ICF components and the categories in the ICF Hand Core Set. 100 patients (86 women, 14 men) with average age of 49.93 ± 11.38 years were included in this study who had RA diagnosis. Disease activity was evaluated by use of DAS28 and disability levels were assessed by DASH, MHQS, DEİ ve AIMS2 scales.The items of the disability assessment scales by the three experts were associated with the categories of the ICF Hand Core Set. There were a moderate correlation between DAS28 scores with DASH (r: 0.643) and DEI (r: 0.575) total scores. There was a relationship between the DAS28 score with the MHQS total score and subscales ranging from low to moderate (r: -0.399 and r: -0.696); between DAS28 and AIMS2 subscales ranging from low to moderate (r:0.220 and r: 0.638). When the level of agreement between the three researchers who associate each item of the disability assessment scales with the ICF components and and the categories in the ICF Hand Core Set was examined, it was seen that the Kappa value varied from 0.78 to 0.90 and was at a high level. When the percentages of activities and participation coverage of item of the disability assessment scales according to the ICF Hand Core Set were examined, it was determined that AIMS2 had the highest percentage as 52.63%. The results of this study indicate that the level of impairment in patients with RA affects the level of disability and activity and participation components in the ICF Hand Core Set are mostly covered by AIM2 among the disability assessment scales.
