Hemşirelerin im enjeksiyon uygulamasında dorsogluteal ve ventrogluteal bölge seçimi ile Z tekniği ile ilgili bilgi ve uygulama sıklığında eğitimin etkinliğinin değerlendirilmesi

Abstract

Araştırma hemşirelerin İM enjeksiyon uygulamasında Dorsogluteal ve Ventrogluteal bölge seçimi ile Z tekniği ile ilgili bilgi ve uygulama sıklığında eğitimin etkinliğinin değerlendirilmesi amacıyla tanımlayıcı ve yarı deneysel olarak planlandı ve gerçekleştirildi. Araştırmanın örneklemini Denizli ilinde bir üniversite hastanesinde, İM enjeksiyon işlemini uygulayan servis ve yoğun bakımda çalışan 169 hemşire oluşturmuştur. Araştırmada veriler, araştırmacı tarafından literatürden yararlanılarak hazırlanan veri toplama formu aracılığı ile toplanmıştır. Veri toplama formunda hemşirelerin tanıtıcı özellikleri, Z teknik ve VG bölgeye İM enjeksiyon ile ilgili çoktan seçmeli sorular, hemşirelerin İM enjeksiyonda DG, VG bölge ve Z tekniğini tercih etme nedenlerini içeren önermeler, hemşirelerin VG bölge ve Z tekniğini tercih etme nedenleriyle ilgili açık uçlu sorular ile İM enjeksiyonda VG, DG ve Z tekniğini eğitim öncesi ve sonrası uygulama sıklıklarını içeren anket formu kullanılmıştır. Araştırmada anket formları hizmet içi eğitim programı öncesi ve sonrasında uygulandı. Eğitim programının hemen sonrasında eğitime katılan ve katılmayan tüm hemşirelere araştırmacı tarafından hazırlanan eğitim kitapçıkları dağıtılmıştır. Eğitime katılanlara 1 ay sonra tekrar izlem anket formları uygulandı. Veriler SPSS paket programıyla analiz edildi. Sürekli değişkenler ortalama ± standart sapma ve kategorik değişkenler sayı (yüzde) olarak verildi. Verilerin değerlendirilmesinde Cochran?s Q Testi, McNemar Testi, Mann Whitney U testi, Kruskal Wallis, Friedman Testi, Marjinal Homojenlik testi, Ki-kare analizi kullanıldı. Katılımcıların eğitim öncesinde bilgi sorularında eksikliklerinin olduğu, eğitim sonrasında ise bilgi düzeylerinde anlamlı oranda artışın olduğu tespit edildi. Z Teknik ve VG Bölge ile ilgili bilgi sorularına verilen doğru cevapların toplam puan ortalaması eğitim öncesi, eğitim sonrası ve izlem arasında istatistiksel olarak anlamlı bir fark saptanmıştır (p<0,05). Hemşirelerin izlemde İM enjeksiyonda tercih ettikleri bölgeler açısından, VG bölgeyi (%68,6) tercih etme oranı artmıştır ve aradaki fark anlamlı bulunmuştur (p<0,05). Hemşirelerin İM enjeksiyonda Z tekniği (%68,6) yöntemini kullanma durumu izlemde, eğitim öncesine göre artmıştır. Eğitim sonrası hemşirelerin VG, DG ve Z Tekniğini ilişkin önermelere verdikleri cevaplarda önemli ölçüde değişim olmuştur ve VG bölgeye ilişkin önermelerde olumlu yönde değişimler olmuştur. Hemşirelerin İM enjeksiyon konularındaki bilgi ve becerileri hizmet içi eğitim programlarıyla geliştirilmeli, eğitim sonrasında belirli periyotlarla bilgilerin güncel tutulması sağlanmalıdır.

Research was planned and carried out as descriptive and quasi-experimental to assess effectiveness of education about knowledge and frequency of Dorsogluteal and Ventrogluteal site selection and Z technique on nurses? IM injection practices. Sample of this research consists of 169 nurses employed in intensive care unit and units which practice IM injection to adults in a university hospital located in Denizli. Data used in this research were collected via data collection form (questionnaire) benefited from the literature by the researcher. Data collection forms consist of descriptive characteristics of nurses, multiple-choice questions about Z technique and IM injection to Ventrogluteal site, propositions including reasons of choosing Dorsogluteal, Ventrogluteal site and Z technique on IM injection, open-ended questions related with reasons of choosing Ventrogluteal site and Z technique, questionnaire about the frequency of VG, DG and Z technique practices before and after the education. Through the research, questionnaires were applied before and after the in-service training program and booklets about the topic prepared by the researcher were distributed to the whole sample right after the training program. After one month follow-up questionnaires were applied to the nurses who attended the training program. SPSS Statistics program was used for data analysis. Percentages, averages, Cochran?s Q Testi, McNemar Test, Mann Whitney U test, Kruskal Wallis, Friedman Test, Marjinal Homegenility test, Chi-square were used to assess data. Through the research, it has been identified that most of the participants have deficiency in questions related with knowledge and the level of knowledge increases significantly after the training. It also has been identified that there exists statistically significant difference (p<0,05) among the mean score of answers to questions asked before training, after training and questions in follow-up questionnaires related with Z technique and Ventroglual site. The nurses? preference rate of Ventrogluteal site on IM injection practices has increased (%68,6) during follow-up and the difference is identified as statistically significant (p<0,05). And also the nurses' preference rate of Z technique on IM injection has increased (%68,6) during follow-up. Nurses? answers to the propositions about reasons of choosing VG, DG and Z technique has changed dramatically after training and the propositions about VG site has changed positively. As a result, nurses' level of knowledge and skills should be developed by means of in-service training and their knowledge should be updated in certain periods.
