Tamsayılı hedef programlama ve hemşire çizelgeleme problemi için bir uygulama

Abstract

İçinde bulunduğumuz teknoloji çağında yaşanan gelişmeler ve makineleşme, farklı sektörlerdeki çeşitli girdileri saf dışı bıraksa bile sunulan hizmetin doğası gereği hizmet seviyesinin mümkün olan en üst düzeyde ve sürekli sağlanması gereken sağlık sektörü, insan gücüne bağımlıdır. Bu sektörde gerekli bilgi, beceri ve yeteneğe sahip personel sayısının az olması, yüksek personel maliyetleri ve tıptaki ilerlemeler planlama ve çizelgelemeyi zorunlu hale getirmektedir. Çizelgeleme yöntemlerinin ekstra katma değer yarattığı endüstrilerin başında yer alan sağlık sektöründe yoğun iş yükü altında hasta ve doktor arasında bir köprü olan hemşirelerin verimliliğinin arttırılabilmesi için adaletli çalışma koşulları sağlanmalı ve hemşire istekleri de göz önünde bulundurularak çizelgeleme yapılmalıdır. Denizli’de bir hastanenin çocuk acil bölümünde yapılan bu çalışmada, ilgili birimde çalışan hemşirelerin 2019 yılının haziran, temmuz ve ağustos olmak üzere 3 aylık periyodundaki vardiya çizelgesini oluşturmaya yönelik tamsayılı hedef programlama modeli önerilmiştir. Önerilen model ile yasal kısıtlar, hemşire ve hastane yönetiminin ortak kararları göz önünde bulundurularak ele alınan periyotta her gün için kesintisiz hizmet sağlanması, fazla mesaiden kaynaklanacak maliyetlerin düşürülmesi, toplam mesai saatleri, gece-gündüz vardiya sayıları ve hafta sonu vardiya sayıları açısından mümkün olan en adaletli çizelgeleme oluşturularak hemşire motivasyonunu da yüksek tutmak amaçlanmıştır.

Despite the developments and mechanization in the current technology age eliminate the inputs in different sectors, due to the nature of the service provided, the health sector which must be provided at the highest possible level and continuously, depends on manpower. The low number of personnel with the necessary knowledge and skills in this sector, high personnel costs and advances in medicine necessitate planning and scheduling activities. Health sector is one of the leading industries where scheduling methods create extra added value. In order to increase the efficiency of nurses, that serve as a bridge between the patient and the doctor with intense work pressure; fair working conditions should be provided and scheduling should be done by considering the requests of the nurses. In this study, which was conducted in the pediatric emergency department of a hospital in Denizli, an integer goal programming model was proposed in order to form the shift schedule of the nurses in the related department in the 3 month periods (June, July and August) of 2019. With the proposed model, considering legal restrictions, joint decisions of nurse and hospital administration; it was aimed to provide uninterrupted service for everyday in the period covered, reduction of overtime costs, total working hours, number of day and night shifts and the distribution of weekend shifts. Hence nurse motivation is kept high by forming the fairest schedule possible.
