Başparmak tendon yaralanmalı hastalarda kavrama paternlerinde meydana gelen değişikliklerle üst ekstremite fonksiyonel durumunun incelenmesi

Abstract

Bu çalışmanın amacı başparmak tendon yaralanmalı hastaların kavrama paternlerindeki değişimi ve bu değişimin üst ekstremitenin fonksiyonel düzeyi üzerine olan etkisini incelemekti.Çalışmaya başparmak tendon yaralanmalı 12 hasta olgu ve hasta olgular ile benzer demografik özelliklere sahip 12 sağlıklı olgu dahil edildi. Olguların yaş ortalamaları 37,62±12,08 yıldı. Olgularda kavrama paternlerinde yüzey alanını değerlendirmek için SolidWorks® CAD programı kullanıldı. Hasta olgularda başparmak fonksiyonlarını değerlendirmek için Geldmacher Skorlama Sistemi, üst ekstremite fonksiyonelliğini değerlendirmek için ise Kol, Omuz ve El sorunları Ölçeği Kısa Formu’nun Türkçe sürümü (Q-DASH) ve Sollerman El Fonksiyon Testi kullanıldı. Hasta grup postoperatif erken dönemde el rehabilitasyonu programına alındı.Çalışmada, hasta olguların kavrama paternlerinde el volar yüzey alanı ile sağlıklı olgulardan elde edilen referans değerler arasında 7. haftada ekstansiyon tip kavrama paterninde, başparmak volar yüzey alanları için 7. haftada pulpa tutuşu kavrama, 12 ve 18. haftalarda küçük çaplı silindirik kavrama paterninde istatistiksel olarak anlamlı fark saptandı (p<0,05). Diğer parametreler arasında istatistiksel olarak anlamlı fark yoktu (p>0,05). Kavrama paternlerindeki değişim ile bazı fonksiyonel ölçüm parametreleri arasında anlamlı ilişki bulundu (p<0,05).Başparmak tendon yaralanmasına sahip hastalarda kavrama paternlerinin değişiklik gösterdiği ve bu değişikliğin de üst ekstremite fonksiyonel düzeyi üzerine etkili olduğu sonucuna varıldı. Bu çalışmada kavrama paternlerinin incelenmesinde objektif bir metod geliştirildi. Herhangi bir yaralanma sonucu ortaya çıkabilecek kavrama paterni değişikliklerinin bu yöntem ile değerlendirildiğinde fonksiyonel durum ile ilişki kurmada klinisyeni objektif sonuca ulaştıracağı sonucuna varıldı.

The aim of this study was to investigate the changes in grasping patterns of the patients with tendon injury of thumb and its effect on the functional status of upper extremity.Twelve patients with tendon injury of thumb and 12 healthy subjects with similar demographic characteristics were included in the study. The mean age of the cases was 37,62±12,08 years. SolidWorks® CAD program was used to evaluate the surface area in grasping patterns. Geldmacher scoring system was used to evaluate thumb functions in the patient group. Turkish version of the short form of Disabilities of the Arm, Shoulder, and Hand questionnaire (Q-DASH) and Sollerman hand function test were used to evaluate the upper extremity function. The patient group was included the hand rehabilitation program in the early postoperative period. In the study, except the 7th week ETK pattern, there was no significant difference between the patients' surface area in volar side of hand and the reference values obtained from the healthy subjects (p>0,05). There was a statistically significant difference in surface area at the pulp pinch pattern in 7th week, at cylindrical grasp (small diamater) patterns in 12th and 18th weeks (p<0,05). There were no statistically significant differences between the other parameters (p> 0.05). There were significant correlations between the changes in grasp patterns and some functional measurement parameters (p <0.05). It has been concluded that the grasping patterns may change in patients with tendon injury of thumb and this change has an effect on the functional status of upper extremity. In this study, an objective method was developed for the investigation of grasping patterns. In conclusion, it was also obtained an objective results in investigation of relationship between grasping patterns and functional status.
