Total diz protezi ameliyatı geçiren hastalarda basınçlı soğuk uygulamanın ağrı düzeyine etkisi

Abstract

Araştırma, total diz protezi ameliyatı sonrası uygulanan basınçlı soğuk uygulamanın ağrı ve memnuniyet düzeyine etkisini incelemek amacıyla randomize kontrollü deneysel bir çalışma olarak yapılmıştır. Bu araştırma Temmuz 2019-Şubat 2020 tarihleri arasında Pamukkale Üniversitesi Hastaneleri Ortopedi ve Travmatoloji Kliniği’ne başvuran, dâhil edilme kriterlerine uyan ve çalışmaya katılmayı kabul eden 52 hastada gerçekleştirilmiştir. Hastalar basit randomizasyon yöntemiyle kontrol grubunda (soğuk jel) 26, çalışma grubunda (basınçlı soğuk terapi bandajı) 26 olacak şekilde ayrılmıştır. Kontrol ve çalışma gruplarındaki hastalara rutin analjezik uygulamasına devam edilmiştir. Kontrol grubundaki hastalara ameliyat sonrası 20 dakika uygulama, 40 dakika dinlendirme şeklinde 9 soğuk uygulama yapılmıştır. Çalışma grubundaki hastalara 30 mmHg basınçla 20 dakika uygulama, 40 dakika dinlendirme şeklinde 9 soğuk uygulama yapılmıştır. Verilerin toplanmasında Kişisel Bilgi Formu ve Visual Analog Skala kullanılmıştır. Verilerin değerlendirilmesinde sayı, yüzdelik, ortalama, standart sapma, Bağımsız Gruplarda t testi, Mann-Whitney U testi, Wilcoxon Eşleştirilmiş İki Örnek testi ve Ki kare Analizi kullanılmıştır. Araştırma sonucunda, çalışma grubundaki 9 uygulamanın öncesi ve sonrası karşılaştırıldığında ağrı düzeylerinde anlamlı farklılık bulunmuştur (p<0,05). Hastaların ameliyat sonrası ilk fizyoterapi sonrası 5. soğuk uygulama ve ameliyat sonrası ikinci gün uygulanan 8. uygulama öncesi ve sonrası çalışma grubunda ağrı düzeyinin daha düşük olduğu saptanmıştır (p<0,05). Kontrol grubundaki hastaların 6. ve 9. soğuk uygulamasında, çalışma grubuna göre ağrı düzeyinde daha fazla azalma olduğu bulunmuştur (p<0,05). Her iki grupta 7. soğuk uygulamada ağrı düzeyinde azalmanın, 1, 3, 5 ve 9. uygulamadaki azalmadan; 9. uygulamadaki azalmanın 1. uygulamadan daha fazla olduğu belirlenmiştir (p<0,05). Grupların son ağrı düzeyleri karşılaştırıldığında, çalışma grubundaki hastaların ağrı düzeyi daha düşük bulunmuştur (p<0,05). Her iki uygulamada memnuniyet düzeyleri arasında anlamlı fark bulunmamıştır (p>0,05). Sonuç olarak çalışma grubundaki hastaların son ağrı ölçümleri daha düşük bulunmuştur.

The study was conducted as a randomized controlled experimental study in order to determine the effect of pressurized cold application applied after total knee replacement surgery on the level of pain and satisfaction. This study was conducted in 52 patients who applied to Pamukkale University Hospitals Orthopedics and Traumatology Clinic between July 2019 and February 2020, who met the inclusion criteria and accepted to participate in the study. The patients were divided into 26 in the control group (cold gel) and 26 in the study group (compressed cold therapy bandage) using a simple randomization method. Routine analgesic administration was continued to the patients in the control and study groups. The patients in the control group received 9 cold applications, in the form of 20 minutes of application and 40 minutes of rest after surgery. The patients in the study group were applied 9 cold applications with 30 mmHg pressure for 20 minutes and 40 minutes of rest. Study data were collected using Personal Information Form and Visual Analog Scale. Number, percentage, mean, standard deviation, Independent Groups t test, Mann-Whitney U test, Wilcoxon Paired Two Samples test and Chi-square Analysis were used for statistical analysis of the data. As a result of the research, a significant difference was found in pain levels when compared before and after 9 applications in the study group (p<0,05). It was determined that the pain level of the patients was lower in the study group before and after the 5th post-operative physiotherapy cold application and the 8th application applied on the second postoperative day (p<0,05). In the 6th and 9th cold applications of the patients in the control group, it was found that the pain level decreased more than the study group (p<0,05). In both groups, the decrease in pain level in the 7th cold application was compared to the decrease in the 1st, 3rd, 5th and 9th applications; It was determined that the decrease in the 9th application was more than the 1st application (p<0,05). When the final pain levels of the groups were compared, the pain level of the patients in the study group was found to be lower (p<0,05). There was no significant difference between satisfaction levels in both applications (p>0,05). In conclusion, the final pain measurements of the patients in the study group were lower.
