45 yaş ve üstü dejeneratif artrit ile birlikte menisküs lezyonu olan hastalarda artroskopik menisektominin değerlendirilmesi (fonksiyonel sonuçlar)

Abstract

Pamukkale Üniversitesi Tıp Fakültesi, Ortopedi ve Travmatoloji Anabilim Dalında Ocak 2001 -Aralık 2004 tarihleri arasında dejeneratif artrit ile birlikte menisküs yırtığı nedeniyle artroskopik menisektomi uygulanan, 6- 50 ay sonra değerlendirilen 139 hastanın fonksiyonel skorları Lysholm Skalasına göre değerlendirildi. Bu çalışma ile dejeneratif artritli hastalarda uygulanan artroskopik menisektominin, dejeneratif artriti daha az derece olanlarda, vücut kitle indeksi daha az olanlarda ve semptom süresi daha kısa olanlarda daha faydalı olduğu ve parsiyel menisektominin subtotal veya total menisektomiye göre daha yararlı olduğu gösterildi. Artroskopik menisektomi, düşük morbidite ve komplikasyon oram ile dejeneratif artrit ve menisküs yırtığı olan seçilmiş hasta gruplarında tercih edilebilecek bir tedavi yöntemidir. Menisküs yırtıklarının tedavisinde, önemli fonksiyonları nedeniyle parsiyel menisektomi gibi menisküsleri korumaya yönelik girişimlerin tercih edilmesi gerektiği kanısındayız. SUMMARY Between January 2001 and December 2004, in the Department of Orthopaedics and Traumatology of Pamukkale University, Faculty of Medicine, 139 patients who underwent arthroscopic meniscectomy for meniscal tear with degenerative arthritis, after 6-50 months whose functional scores were evaluated by Lysholm's scales. This study showed that arhroscopic meniscectomy were applied to the patients who had degenerative arthritis; the patients who had lower degree of degenerative arthritis, lesser body mass index and shorter duration of symptoms had more benefits and partial meniscectomy is more beneficial than the subtotal or total meniscectomy. Arthroscopic meniscectomy with low morbidity and complication rate is preferable treatment method for selected patients with degenerative arthritis and meniscal tear. Since the functions of the meniscii are very important, procedures like partial meniscectomy should be prefered in meniscal tear treatment.
