Türkiye’de kamu sağlık sektöründe personel rejimi, 1923–2023: Dönüşümler, politikalar, sorunlar ve bakış açıları

Abstract

Türk Kamu Personel Rejiminin yolculuğu Cumhuriyetin kurulmasından günümüze kadar oldukça farklı ilerlemiştir. Reformlarla, kanunlarla ve çalışmalarla eksiklik görülen hususlar düzeltilmeye çalışılmıştır. Özellikle kamu hizmeti alanında geniş yer tutan sağlık hizmetlerinin sunumunu gerçekleştiren, sağlık hizmetlerinin görünür yüzü kamu sağlık personeli de değişiklik, yenilik ve reformlardan payına düşeni fazlasıyla almış; ayrıca çok da farklı şekillenmiştir. Sağlık hizmetinin sunumundaki zorluklar ve hizmet çeşitliliği sağlık personelinde farklı istihdam türlerini oluştururken aynı zamanda yeni riskleri, çatışmaları, farkları ve çalışma hayatına dair bazı konuları da gündeme getirmiştir. Bu çalışmada 1923-2023 yılları arasında sağlık personel rejiminin dönüşümü, gelişimi ve bunların nedenleri ve sağlık personeline dair çalışma hayatını etkileyen faktörler, rejim çeşitliliği politikalar ve reformlar doğrultusunda değerlendirilmiştir. Çalışma beş bölümden oluşmaktadır. Birinci bölümde Türkiye’de kamu personel rejimi ve sağlığa dair gelişmelere; ikinci bölümde konuya dair literatür analizine; üçüncü bölümde sağlık personelin tarihsel gelişimini içine alan düzenlemelere; dördüncü bölümde sağlık personeli özelinde planlama, istihdam, farklılıklar, çalışma yaşamına yönelik konulara değinilmiştir. Beşinci bölümde ise konu çerçevesinde Denizli İlinde yer alan sağlık tesislerinde görevli yöneticiler (kamu, özel, üniversite) ve konuya hakim öğretim üyeleri ile yarı yapılandırılmış mülakat temelli bir araştırma yapılmıştır. Katılımcıların cevapları Nvivo analiz programı yardımıyla ve literatür ışığında analiz edilmiş; yorumlanmış ve çözüm önerileri getirilmiştir.

The journey of Turkish Public Personnel Regime has progressed quite differently from establishment of government to present. Lacking issues have been tried to be improved through reforms, laws and studies. Especially the public health personnel, face side of healthcare services, who perform presentation of healthcare services having an extensive coverage in field of public service have taken their share greatly from alterations, innovations and reforms and they have taken shape very differently, as well. Whereas the challenges in the presentation of healthcare system and service range have created different employment types for the healthcare personnel, they have raised new risks, conflicts, differences and certain topics concerning working life at the same time. Within this study, the transformation and development of the healthcare personnel regime from 1923 to 2023, its reasons and factors influencing working life about the health personnel are evaluated in line with regime varieties, policies and reforms. The study consists of five parts. While the first part focuses on the public personnel regime in Turkey and developments concerning health, the second part focuses on literature analysis about the topic. The third part mentions the regulations including historical development of the healthcare personnel and the fourth part refers to planning, employment, differences specific to the healthcare personnel and issues concerning working life. Within the fifth part, a semi-structured interview based research has been carried out with the commissioned managers in the healthcare facilities in Denizli province (public, private and university) within the framework of the topic and the faculty members knowing the stuff. Answers of participants have been analysed with the help of Nvivo analysis programme in the light of literature, interpreted and solutions have been offered.
