dc.description.abstractÇalı?mamızın amacı pulmoner cerrahi sonrası solunum kas eğitiminin etkinliğini incelemekti. Çalı?mamıza ya? ortalamaları 55,55±10,54 olan, pulmoner cerrahi geçiren (28 erkek, 12 kadın) 40 olgu dahil edildi. Olgular randomize olarak iki gruba ayrıldı. Çalı?ma grubundaki olgulara(n=20), postoperatif dönemde günde 1 kez solunum kontrolü, diyafragma solunumu, kostal ekspansiyon egzersizleri, bron?iyal hijyen teknikleri, etkili öksürme, postür egzersizleri ve erken mobilizasyondan olu?an konvansiyonel pulmoner fizyoterapiye ek olarak günde 6 kez solunum kas eğitimi uygulandı. Kontrol grubundaki olgulara(n=20) ise postoperatif dönemde günde 1 kez sadece konvansiyonel pulmoner fizyoterapi uygulandı ve insentif spirometre kullanımı önerildi. Uygulanan tedaviler olgular taburcu olana kadar sürdürüldü. Tüm olguların preoperatif, postoperatif ve taburcu olmadan önce, ağrı düzeyi(VAS), solunum kas gücü (M?P-MEP), dispne düzeyi (MMRC), egzersiz kapasitesi (6DYT), anksiyete-depresyon düzeyi (HAD) değerlendirildi ve hastanede kalı? süreleri belirlendi. Tedavi sonucunda her iki grubun postoperatif dönemde azalan solunum kas gücünün taburcu olmadan önce arttığı, bu artı?ın çalı?ma grubunda anlamlı derecede daha fazla olduğu belirlendi (p<0,05). Çalı?ma grubu olgularının hastanede kalı? sürelerinin kontrol grubuna göre anlamlı düzeyde daha kısa olduğu belirlendi.(p<0,05). Ağrı ve anksiyete-depresyon sonuçları açısından iki grup arasında farklılık bulunmadı.(p>0,05). Çalı?mamızın sonuçları pulmoner cerrahi geçiren olgularda, postoperatif dönemde konvansiyonel pulmoner fizyoterapiye ek olarak solunum kas eğitimi uygulamasının solunum kas gücü, dispne ve hastanede kalı? süreleri açısından olumlu sonuçları olabileceğini gösterdi. The aim of our study was to analyze the effectiveness of respiratory muscle training in addition to conventional pulmonary physiotherapy after pulmonary surgery. 28 male, 12 female, totally 40 subjects with a mean age of 55,55 ± 10,54 undergoing pulmonary surgery were included into the study. The subjects were divided into two groups randomly. The subjects who were in the study group (n=20) received respiratory muscle training six times a day added to conventional pulmonary physiotherapy once a day which consisted of control of breathing, diaphragmatic breathing, costal expansion exercises, bronchial hygiene techniques, effective coughing, posture exercises and early mobilization in postoperative period. The subjects who were in control group (n=20) received only conventional pulmonary physiotherapy once a day and were recommended to use incentive spirometry. The treatments were applied until discharge. The pain level (VAS), respiratory muscle strength (MIP-MEP), dyspnea level (MMRC), exercise capacity (6MWT), anxiety-depression (HAD) were measured pre-postoperatively and before discharge. The length of stay at hospital was determined. As a result of treatment, the respiratory muscle strength of subjects in both groups which had been decreased after surgery, was increased before discharge, and the increase was significantly higher in the study group (p<0, 05). The exercise capacity and dyspnea level of subjects which had been decreased postoperatively were returned to preoperative levels in study group whereas they decreased significantly in control group (p<0, 05) before discharge. The mean duration of stay of length at hospital was significantly shorter in study group (p<0, 05). There weren't any differences between groups in terms of pain level and HAD scores. The results of our study shows that addition of respiratory muscle training to conventional pulmonary physiotherapy after pulmonary surgery can have positive effects on respiratory muscle strength, dyspnea, exercise capacity and stay of length at hospital.en_US
