dc.description.abstractAmaç: Pamukkale Üniversitesi Tıp Fakültesi Hastanesinde 2009-2012 yılları arasında takip edilen akut pankreatitli vakaları iredelemek .Gereç ve yöntem: Toplam 134 akut pankreatitli vaka retrospektif olarak irdelenmiştir. 134 vakanın 64?ü erkek (%47), 70?i kadın (%53) cinsiyettir. Erkeklerin yaş ortalaması 59,89±17,82, kadınların yaş ortalaması 62,33±16,94, tüm grubun yaş ortalaması 61,16±17,34 tür. Cinsiyetler arasında yaş anlamında fark yoktur (p>0,05). 134 pankreatit vakasının 104?ü interstisyel ödematöz (%79) pankreatit, 29?u nekrotizan (%21) pankreatit vakasıdır. Biliyer kökenli pankreatitler vakaların %67?sini oluşturmaktadır. Alkole bağlı pankreatit %7.4, hiperlipidemiye bağlı pankreatit %3.7?dir. Ayrıca post-ERCP (%1.4) post-op ve post travmatik (%6.4), ilaca bağlı ve obeziteye bağlı (her biri %1.4) ve diğer nedenlere bağlı i (%11.1) pankreatitli vakalar saptanmıştır. Ödematöz pankreatitli grubun Ranson skor ortalaması 1,35±1,11 nekrotizan pankreatitli vakaların Ranson skor ortalaması 1,95±1,32? dir (p=0,034). Baltazar şiddet indexi ödematöz pankreatite 3,55±0,90 nekrotizan pankreatite 5,78±1,63?tür (p=0,0001). Hastaların ortalama yatış süresi ödematöz grubun 7,74 ±4,50 gün nekrotizan pankreatitli grubun 14,65±9,85 gündür (p=0,0001). Hastaların tanı alıp tedavisine başlamaya kadar olan süre ödematöz grupta 3,08±3,38 gün nekrotizan grupta 7,17±7,92 gündür (P=0.001).Vaka serimizde mortalite oranı %5,2? dir. Ödematöz pankreatitli bir vakanın ortalama maliyeti 2.238,00 TL, nekrotizan pankreatitli bir vakanın ortalama maliyeti 4.4452,00 TL?dır (P=0,001). Sonuç: Pankreatitli vakayı erken tanıyıp erken tedavi etmek prognozu olumlu etkileyebilir.en_US
dc.description.abstractAim: The aim of the study is to investigate the clinical aspects of patients who were followed up with diagnosis of acute pancreatitis in Pamukale University Hospital between 2009 and 2012. Material and Method:134 patients diagnosed as acute pancreatitis were investigated retrospectively. 64 of 134 patients (%47) were male and 70 of them (%53) were female. Average age is 61,16±17,34, and is 59,89±17,82 and 62,33±16,94 for male and female patients respectively. This did not show any significant difference (p>0.05). 104 of 134 cases (%79) were edematous pancreatitis while 29 of them (%21) were necrotising. Biliary ethiology was assessed in %67 of the cases. The other ethiological factors were as follows; alcohol induced (%7.4), hyperlipidemia (%3.7), post-ERCP (%1.4), postoperative or posttravmatic (%6.4), drug induced (%1.49, obesity associated (%1.4) and other reasons (%11.1). The mean Ranson scoring for edematous group was lower (1.35±1.11) when compared to the necrotising group (1.95±1.32) and the difference was statistically significant (p=0.034). Balthazar severity index was also significantly different between these two groups (3,55±0,90 vs 5,78±1,63, p=0,0001). The mean hospital stay for edematous and necrotising group was 7,74 ±4,50 days and 14,65±9,85 days respectively, which was also significantly different (P=0.001). The mortality rate in our case series is %5.2. The mean cost for the treatment of a patient with edematous pancreatitis was found to be 2,238.00 TL. This cost was 4,452.00 TL for a patient with necrotising pancreatitis (P=0,001). Result: Early recognition and treatment for a patient with pancreatitis can improve prognosis.en_US
