Eksudatif tip yaşa bağlı makula dejenerasyonundaki koroid neovaskülarizasyonunun tedavisinde intravitreal anti-VEGF uygulamalarının karşılaştırılması

Abstract

Bu çalışmada amaç, eksudatif tip yaşa bağlı makula dejenerasyonuna ikincil olarak gelişen koroid neovaskülarizasyonunun tedavisinde intravitreal anti-VEGF uygulamalarının etkinliklerinin karşılaştırılması idi. Ocak 2015 - Eylül 2017 arasında kliniğimizde eksudatif tip yaşa bağlı makula dejenerasyonu nedeniyle değerlendirilen, intravitreal ranibizumab veya aflibercept uygulanan hastalar çalışmaya dahil edilerek başlangıçta ve tedavi sonundaki Snellen eşeli ile görme keskinlikleri(logmar’a çevrilmiştir), optik koherans tomografi ve fundus floresein anjiografi görüntüleri retrospektif olarak hasta kayıtlarından değerlendirildi. 50 göze intravitreal ranibizumab, 50 göze intravitreal aflibercept uygulanmıştı. Ortalama izlem süresi ranibizumab grubunda 13.62 ay, aflibercept grubunda 11.24 aydı. Tedavi sonrası ortalama görme düzeyinde tedavi öncesine göre ranibizumab grubunda artma (p= 0.0001) ve aflibercept grubunda’da artma saptandı (p= 0.0001). Ortalama görme düzeyindeki değişiklik her iki grupta da istatistiksel olarak anlamlı olarak bulundu. Tedavi sonrası ortalama santral makular kalınlıkta tedavi öncesine göre ranibizumab grubunda azalma (p=0.0001) ve aflibercept grubunda’da azalma saptandı (p=0.0001). Ortalama santral makular kalınlıktaki düşüş her iki grupta’da istatiksel olarak anlamlı olarak bulundu. İntravitreal ranibizumab ve aflibercept uygulamasına bağlı komplikasyon görülmedi. Çalışmamızda intravitreal anti-VEGF uygulamalarının eksudatif tip yaşa bağlı makula dejenerasyonlarındaki koroid neovaskülarizasyonunun tedavisinde etkili olduğunu ve bu çalışma grubunda iki tedavinin sonuçlarının benzer olduğunu saptadık.

The aim of this study was to compare the efficacies of intravitreal ranibizumab and aflibercept for the treatment of choroidal neovascularization due to age related macular degeneration. The study group consisted of patients with exudative age related macular degeneration, attended to our clinic between January 2015 and September 2017 and were treated with intravitreal ranibizumab or aflibercept. For all patients, data of visual acuity measured with Snellen chart (converted to logMAR), optical coherence tomography and fundus fluorescein angiography at the beginning and at the end of intravitreal therapies were evaluated. Fifty eyes were treated with intravitreal ranibizumab, fifty with intravitreal aflibercept. Mean follow up was 13.62 months for ranibizumab group, 11.24 months for aflibercept group. Mean visual acuity after the treatment increased for the ranibizumab group (p=0,0001), increased for the aflibercept group (p=0,0001). The difference between mean visual acuity values at the end and at the beginning of the study in each group was statistically significant. Mean central makular thickness after the treatment decreased for the ranibizumab group (p=0,0001), decreased for the aflibercept group (p=0,0001). The difference between mean central makular thickness values at the end and at the beginning of the study in each group was statistically significant. No complications related to intravitreal anti-VEGF application were observed. We found that intravitreal ranibizumab and aflibercept were effective for the treatment of choroidal neovascularization in exudative age related macular degeneration and the results of the two treatments were similar in this study group.
