Hodgkin lenfoma hastalarında F-18 FDG pet metabolik parametrelerinin tedaviye yanıtı değerlendirme ve prognozu öngörmedeki yeri

Abstract

Çalışmamızın amacı, tedavi öncesi, tedavi sonrası ve interim F-18 FDG PET/BT’ten elde edilen MTV ve TLG gibi tümör yükünü gösteren volumetrik metabolik PET parametrelerin HL hastalarında progresyonsuz sağkalım (PFS) ve genel sağkalım (OS) için prediktif değerini retrospektif olarak araştırmaktır.Bu çalışmada histopatolojik olarak Hodgkin Lenfoma tanısı almış erişkin yaş grubunda 63 hastanın tedavi öncesi, tedavi sonrası ve interim PET/BT’lerinden MTV ve TLG değerleri hesaplandı. İstatistiksel yöntemlerle MTV ve TLG’nin progresyonsuz sağkalım ve genel sağkalıma etkisi araştırıldı. Progresyonlu hastaların tedavi öncesi MTV1, interim PET MTV2 ve TLG2, tedavi sonrası MTV3, TLG3 değerleri progresyonsuz hastalara göre anlamlı olarak daha yüksekti (p<0,05). Yaşayan hastaların tedavi öncesi MTV1, TLG1, tedavi sonrası MTV3, TLG3 değerleri ölen hastalara göre daha düşüktü ve fark istatistiksel olarak anlamlıydı (p<0,05). Cox regresyon analizine göre tedavi öncesi MTV1, TLG1, tedavi sonrası MTV3 ve TLG3, PFS üzerine anlamlı düzeyde etkiliydi. Cox regresyon analizinde MTV1, TLG1, MTV2, TLG2, MTV3, TLG3 OS üzerine anlamlı düzeyde etkiliydi. ROC analizinde, yaşayanlarla ölenler arasında MTV1 açısından kesme değeri 308 cm3, progresyonlu ve progresyonsuz hastalar arasında MTV1 açısından kesme değeri 232 cm3 olarak bulundu. Hodgkin lenfoma hastalarında FDG-PET’den elde edilen MTV ve TLG gibi metabolik volumetrik parametrelerin PFS ve OS’yi öngördüğünü saptadık.

The aim of our study is to retrospectively evaluate to predictive values of volumetric metabolic PET parameters, such as MTV and TLG, which obtained from pre-treatment, post-treatment and interim F-18 FDG PET/CT for progression-free survival (PFS) and overall survival (OS) in HL patients.In this study, MTV and TLG values were calculated from the pre-treatment, post-treatment and interim PET/CTs of 63 adult patients diagnosed with Hodgkin lymphoma histopathologically. The predictive values of MTV and TLG in progression-free survival and overall survival were evaluated by statistical methods.Pre-treatment MTV (MTV1), interim MTV (MTV2) and TLG (TLG2), post-treatment MTV (MTV3) and TLG (TLG3) values were significantly higher in patients with progression compared to without progression (p<0.05). The MTV1, TLG1, MTV3 and, TLG3 values of the surviving patients were significantly higher (p<0.05). According to Cox regression analysis, MTV1, TLG1, MTV3 and TLG3 were statistically significant for PFS and MTV1, TLG1, MTV2, TLG2, MTV3, TLG3 were statistically significant for OS. In the Roc analysis, the cut-off value of MTV1 was found as 308 cm3 for OS, and 232 cm3 for PFS. We found that metabolic volumetric parameters of FDG PET, such as MTV and TLG, predicted PFS and OS in HL patients.
