    Does early or late discharge after total knee replacement affect the burden and stress of caregivers?

    Background: Since patients may experience difficulties in their daily activities after total knee replacement surgery, the role of the caregiver is important in supporting the daily needs of the patients. Caregivers are involved in the day-to-day care activities of the patient during the recovery process, managing their symptoms and providing support. All these factors can affect the burden and stress of caregivers. Methods: It was aimed to compare the caregiver burden and stress of caregivers of total knee replacement patients who were discharged on the same day of surgery and at a later point. Data were collected from 140 caregivers by using the Bakas Caregiving Outcomes Scale, Zarit Caregiving Burden Scale, and Stress Coping Styles Scale. Results: There was no significant difference between on the same day of surgery discharge and later discharge in terms of care burden and stress of caregivers (p > 0.05). While the burden of care was mild to moderate (22.15 ± 13.76) on the same day of surgery discharge group, the burden of care in the later discharge group was very low (19.03 ± 13.65). Conclusion: In order to reduce the care burden and stress levels of caregivers, it is important that nurses determine the problems related to caregiving and provide the necessary support. © 2023 Elsevier Ltd

    Reasons for individuals' COVID-19 vaccine hesitations and changing decisions over time: A longitudinal qualitative study

    Objective: COVID-19 is a serious public health problem. Therefore, it is important to vaccinate the community. This study was aimed at examining the reasons for individuals' COVID-19 vaccine hesitations, and decisions that have changed over time. Material and methods: This was a prospective longitudinal qualitative study. Results: Confidence, fear, conspiracy theories, belief, and social pressure play a role in vaccine hesitancy. The most important factor in the changing decisions of individuals who are hesitant about vaccination is pressure. Confidence and fear also lead to a change of decision. However, it was remarkable that most of the participants who were vaccinated due to pressure did not want to be revaccinated. Conclusions: Governments, media, vaccine companies, health professionals, and scientists should act together to increase the vaccination rates of individuals. © 2022 Elsevier España, S.L.U.

    Fear of childbirth among pregnant women and their partners in Turkey

    Objective: The aim of this study was to identify and compare the fear of childbirth and associated factors among pregnant women and their partners. Methods: This was a descriptive and relational study. A total of 564 people, 282 women and their partners, were included in the study. Data were collected using two forms: the Personal Information Form and the Fear of Birth Scale. Results: The study found that 82.6% of the women and 54.3% of their partners had a fear of childbirth. Economic status, parity, previous birth experience, and preferred delivery method affected the fear of childbirth among the women (p = 0.000, p = 0.000, p = 0.000, and p = 0.030, respectively). Age, parity, education level, economic status, preferred delivery method, and previous birth experience of their partner affected the fear of childbirth among the men (p = 0.036, p = 0.000, p = 0.000, p = 0.004, and p = 0.000, respectively). A positive, significant, moderate relationship was found between the fear of childbirth among the women and their partners (r = 0.602, p = 0.000). Fear of childbirth affected both the women's and their partners’ preferred delivery method (p = 0.000 and p = 0.005, respectively). Conclusion: Partners had a fear of childbirth as well, although the rate of this fear was higher among women. Fear of childbirth caused the couples to prefer cesarean section. There was a relationship between the fear of childbirth among the women and their partners. © 2020 Elsevier B.V.

    Becoming a family caregiver of a patient living with cancer

    Background: There is insufficient information on how the burden of caregiving is affected when the family caregiver is a health professional. Studies are needed to investigate this issue. Aims: The purpose of this study was to reveal difficulties experienced by a nurse family caregiver offering care to a family member diagnosed with end-stage cancer and how she coped with these difficulties. Methods: This was an autoethnographic study. Findings: Findings were grouped under three headings: Being both a researcher and a subject; effects of caregiving; and coping. Conclusions: Offering care to a cancer patient has many physiological and psychological effects. If a family caregiver is a health professional, his/her caregiving burden can be even higher. Cultural values affect both life and coping ways of caregivers. It should be kept in mind that family caregivers need support from health professionals whatever their occupations are. Support to caregivers plays an important role in their coping. © 2020 MA Healthcare Ltd. All rights reserved.

    Acute effects of distal fibular taping technique on pain, balance and forward lunge activities in Chronic Ankle Instability

    BACKGROUND: Chronic Ankle Instability (CAI) is characterized by altered muscle activation, reduced range of motion (ROM) and balance deficits. OBJECTIVE: The aim of this study was to determine whether fibular taping technique influenced the pain intensity, dorsiflexion ROM and dynamic balance in participants with CAI. METHODS: Twenty-six participants (16 males and 10 females; age 28.8 - 5.74 y) with unilateral CAI who scored 85 points or less on the Foot and Ankle Outcome Score (FAOS) were included in our study. Mulligan distal fibular taping technique was applied. Visual Analog Scale, Star Excursion Balance Test in anterior, posteromedial and posterolateral directions and a Weight Bearing Lunge Test were taken before, after, 1 hour after and 24 hours after taping. RESULTS: Distal fibular taping technique decreases pain intensity (at rest, during range of motion and weight bearing conditions), while it increases forward lunge distance and reach distance in anterior, posteromedial and posterolateral directions (p = 0.001). CONCLUSIONS: Our study showed that distal fibular taping reduces pain intensity, postural control and dorsiflexion range of motion in individuals with CAI. Further studies are needed with participants with a lower functional level and a higher pain intensity. © 2019-IOS Press and the authors.

    The stress levels of parents of premature infants and related factors in nenoatal intensive care units

    The aim of this study is to determine the stress levels of parents whose premature baby are hospitalized in the neonatal intensive care unit (NICU) and to determine the factors affecting their stress. The study sample included 101 parents of premature babies in the NICU of a university hospital. Interviews were conducted with the parents. Additionally, the Parental Stress Scale: Neonatal Intensive Care Unit (PSS:NICU) was administered. When the stress levels of parents in the NICU were analyzed, it was found that their scores on the infant’s appearance and behaviors subscale of the PSS:NICU reflected the highest rate of stress. The mean score on the parental role alterations subscale was higher among mothers than fathers (p<0.05). Additionally, high stress levels were found among parents who had children other than their baby in the NICU, parents whose baby had a young gestation age and a low birthweight, parents whose baby was on a respiration device, and parents who had experienced a stressful event in the past year. The stress levels of parents whose premature baby was hospitalized in the NICU were also high. The family-centred approach is recommended to identify and meet all the needs of all such parents. © 2018, Turkish Journal of Pediatrics. All rights reserved.

    The effect of theoretical and simulation training on medical errors of nurse students in karadeniz technical university, Turkey

    Objective: To assess the efficacy of theoretical training related to medical errors and patient safety and practical simulation training on students' knowledge and skill levels. Methods: This experimental study was conducted from March 28 to May 18, 2016, at nursing department of a university and comprised of students attending the third grade of the nursing school. The subjects were divided into two equal experimental and control groups. The experimental group, among other things, received simulation training that involved a demonstration and subsequent one-to-one practice. Data was collected on a proforma that included demographic information, pretest and post-test scores, checklists for pre-assessments and final assessments related to nursing practices. Results: There were 62 nurses divided into two groups of 31(50%) each. The difference between post-test scores of the concerning theoretical training about medical errors and patient safety was statistically significant (p<0.05). Pre-assessment of the groups concerning practical training on nursing skills and medical errors revealed no statistically significant difference (p>0.05), while there was a significant difference between their final assessments (p<0.05). Conclusion: Theoretical education increased students' levels of knowledge regarding medical errors and patient safety, and the the practical simulation training reduced malpractices by improving skills. © 2018, Pakistan Medical Association.     Is cold therapy really efficient after knee arthroplasty?

    Introduction: Knee arthroplasty is a frequently used surgery. The purpose of this study is to evaluate the effects of cold therapy after knee arthroplasty. Methods: 60 patients who were operated in our clinic between the years 2013 and 2014 were evaluated prospectively, and the patients were randomized into two groups. The cold therapy by Cryo/Cuff application was started 2 h before the surgery in Group-2 patients; it was repeated in a postoperative 6th hour and continued for postoperative four days. We analyzed the visual analog scales (VAS) for pain, Knee Society Scores (KSS), hemoglobin, and bleeding parameters. Results: The data of the group that did not receive the Cryo/Cuff application (Group-1) are as follows; the number of the patients was 33; the mean age value was 68,4 (53-78). Group-2 data was as follows; the number of the patients was 27; the mean age value was 67.2 years (range 57-78). The mean KSS scores of the patients increased from 79.6 (75-83) and to 90.5 (88-92) postoperatively (p < 0.05). Discussion: In our study, the pain and functional knee scores of the patients who received Cryo/Cuff application were significantly different and as expected, the use of analgesics was much lower. Cryotherapy did not affect the amount of bleeding during the surgery (p > 0.05), which was not reported previously in the literature. Conclusion: After knee arthroplasty, the preoperative and postoperative use of cryotherapy is effective in terms of the pain control and functional knee scores without a significant change in surgical blood loss. © 2015 The Authors.

    The effect of sagittal plane deformities after tibial plateau fractures to functions and instability of knee joint

    PURPOSE OF THE STUDY The objective of this study is to evaluate the effect of posterior tibial slope after fracture healing on antero-posterior knee laxity, functional outcome and patient satisfaction. MATERIAL AND METHODS 126 patients who were treated for tibial plateau fractures between 2008-2013 in the orthopedics and traumatology department of our institution were evaluated for the study. Patients were treated with open reduction and internal fixation, arthroscopy assisted minimally invasive osteosynthesis or conservative treatment. RESULTS Mean posterior tibial slope after the treatment was 6.91 ? 5.11 and there was no significant difference when compared to the uninvolved side 6.42 ? 4,21 (p = 0.794). Knee laxity in anterior-posterior plane was 6.14 ? 2.11 and 5.95 ? 2.25 respectively on healthy and injured side. The difference of mean laxity in anterior-posterior plane between two sides was statistically significant. DISCUSSION In this study we found no difference in laxity between the injured and healthy knees. However Tegner score decreased significantly in patients who had greater laxity difference between the knees. We did not find significant difference between fracture type and laxity, IKDC functional scores independent of the ligamentous injury. CONCLUSION In conclusion despite coronal alignment is taken into consideration in treatment of tibial plateau fractures, sagittal alignment is reasonably important for stability and should not be ignored. © 2016, Galen s.r.o.     Nurses' and paediatricians' knowledge about infant sleeping positions and the risk of sudden infant death syndrome in Turkey

    Objective: The purpose of this article is to determine nurses' and paediatricians' knowledge regarding the sleeping positions and environment of infants. Methods: This descriptive and cross-sectional study was conducted at 18 randomly selected hospitals in eight different cities in Turkey. There were 1,156 participants in the study (252 physicians and 904 nurses). The data were collected by means of a questionnaire form developed by the investigators. Data analysis included percentage and chi-square analyses. Results: Among the nurses, 74.1% were between the ages of 21 and 35, 32.0% had a bachelor's degree, 65.0% had job experience of six years or more. Among the paediatricians, 69.0% were between the ages of 21 and 35; 42.5% had job experience of six years or more. 88.8% of the nurses said that the mother should share same room with the infant but in a separate bed. Over two-thirds of the nurses said that a pillow should not be used when an infant was asleep, and 98.0% would not cover an infant's face. 86.5% of the paediatricians said that the mother could share same room with the infant but in a separate bed. 76.2% of the paediatricians said that they would not use a pillow when the infant were asleep, and 97.2% would not cover an infant's face. Most nurses and physicians responded that infants of 0-6 months of age slept on their sides during the daytime, in the nighttime, when left alone in a room, and after feeding. Conclusions: The results showed that physicians and nurses were not sufficiently knowledgeable about infant sleeping positions; however, they had sufficient knowledge about the risk factors for sudden infant death syndrome. The education of both nurses and physicians working in pediatric wards about the risk factors of SIDS may decrease SIDS deaths in Turkey.
